Évaluation de l'efficacité de l'accouchement en décubitus latéral

Abstract : Objective : To identify the benefits and drawbacks of the lateral decubitus position in the delivery, establishing an inventory once a student at the point of view quantitative and qualitative, and the satisfaction of couples and the midwife.

Methods : Study population Retrospective, single center at the University Hospital of Angers (49), identifying 114 patients who delivered in the lateral decubitus position at March 17, 2005 to June 28, 2010. These deliveries are all made by the same midwife, who since 2005 has identified all deliveries that she had done in lateral decubitus, during his postings in the delivery room. Included were singleton pregnancies of normal, cephalic presentation, who began spontaneous labor at term or artificial.

Results : There was less use of oxytocin, a rate less analgesia and all patients gave birth by spontaneous vaginal delivery. Regarding the perineum, a higher rate of tearing the 1st and 2nd degree was found, however, the episiotomy rate is less, and this very significantly. Furthermore, no tear 3rd or 4th degree was found. A very good adaptation to life for every newborn was found, and none was transferred to neonatal service. There was no significant difference found for the other parameters. Finally, satisfaction was found in all parturients, fathers and among the midwife with the lateral decubitus position during delivery

Conclusion : The lateral decubitus in the delivery is beneficial both in terms of obstetric, maternal, that the foetal perspective. The lateral decubitus position is a prevention easy to implement, and is a simple treatment of minor problem's labor and delivery. Finally, the delivery in lateral decubitus allows women, couples to reclaim their birth, by participating more actively in the birth, and also allows midwives to find their primary function of guaranteeing the eutocic.
