Hypertension Artérielle Résistante : Prise en charge. Revue de la Littérature - Étude de cas

Abstract : Resistant Hypertension : Care Management. Literature Review - Case Study. High Blood Pressure constitutes a major Public Health issue. Recent medical research shows a growing prevalence of Resistant Hypertension, which increases the risks of cardio-vascular related mortality. The aim of this work is to make recommendations for care management and the treatment of Resistant Hypertension, and to present an up to date review of the literature concerning diagnosis and treatment. The case study carried out in the department of Cardiology in the CHRU ( Centre Hospitalier Regional Universitaire) of Brest, from 01/01/2008 to 15/03/2011 enables us to evaluate the diagnosis and care management and analyses 4 cases of Resistant Hypertension. These correspond to the established guidelines and are secondary to primary aldosteronism, renal artery stenosis, chronic kidney disease, which are the most frequent causes of Resistant Hypertension. The low number of patients detected indicates that Resistant Hypertension is underestimated. An optimal management by the physician is necessary to eliminate the problems of non -adherence or particular factors such as excess salt intake, excessive alcohol intake, obstructive sleep apnea before considering referral to a specialist for secondary hypertension screening.
