La Communauté Hospitalière de Territoire : face à un exercice de territorialité, quelle mutation culturelle pour un établissement de santé ?

Abstract : In France, since the creation of health insurance in 1945, the rise in health spendings regularly increases the gross domestic product forcing the public authorities to generate additional funding and operational savings while avoiding jeopardizing the quality of care. Since a decade, health policy opted for a reorganization of provision of care within the health territories. This reorganization leads to clustering health institutions in the perspective of better cooperation and pooling resources. The example of the hospital of Crest is emblematic of this exercise of territoriality. The Regional Agency of Health orders the establishment to expand its offering of care within the Community Hospital of Territory Centre-Nord-Drôme. The objective of this study is to describe the process of change taking place within the structure, and to understand the possible cultural and identity change in the making. It appears that the Community Hospital Territory generates a cultural change that occurs in rebalancing the professional relationships and working standards. Promote a culture of change requires to consider human, organizational and technical aspects... It implies having a strong desire in terms of management, and taking into account the representations of the actors towards the change.
