Bankart arthroscopique de patients âgés de plus de 30 ans. Résultats cliniques et radiologiques à plus de 10 ans

Abstract : Few long‐term studies have evaluated the clinical outcomes of patients treated for shoulder instability with arthroscopic Bankart with 10 years' follow‐up minimum . The aim of the study was to assess the failure rate of this technique at 10 years follow up and the risk factors in patients after 30 years old. Fifty‐two patients operated for anterior chronic shoulder instability with arthroscopic Bankart between 1999 and 2003 were included. Forty‐one patients have been evaluated (79%) with a mean follow up of 12,51 months. We analysed retrospectively the recurrence of the lesion based on ISIS score and bone loss (glenoid and humeral head), clinical and functional outcomes (Duplay-Walch and ISIS scores) and rates of arthritis. Recurrent dislocations or subluxations were considered as treatment failure. Fifteen patients (37%) had recurrent instability. Mean age of patients with recurrent instability was 3 years less than patient without recurrence. 83 % of patients with a preoperative ISIS score > 2 (p=0,001) and 55% of patients with glenoid bone loss (p=0,09) had recurrent instability. All the patients with a Hill Sachs lesion > 20% had recurrence (p<0,001), and 38% of patients presented shoulder arthritis. Patients without recurrence had better results for Duplay‐Walch and WOSI scores. None of the patients with an ISIS score ≤ 2, a Hill Sachs lesion ≤ 15% and no glenoid bone loss had recurrence.
