Analyse de l’équilibre et de la marche avant et après ostéotomie de valgisation pour gonarthrose

Abstract : Gonarthrosis with varus alignment leads to balance and gait disorders. Correcting this axis, osteotomy changes these factors. The aim was to analyse the patient's perception of balance and gait, and by comparing to objective analysis, before and after 4 months of osteotomy. Our hypothesis was an improvement of gait and balance parameters but a perception of a worse gait and balance. Twenty four patients aged 52.4 years +/- 6.8, obese (index of body mass: 31.2 +/- 5.4), were paired by sex and age to controls. Deficiencies and disabilities were evaluated by means of questionary: numeric scale, Lysholm score, SF-36 score, Koos and Womac score. From a 4 levels scale: very bad, bad, good and very good, patients and controls estimated their perception of balance and gait. Objective balance analysis (from a balance platform) and gait analysis (from a bidimentional gait analysis) were achieved. Our hypothesis hasn't been checked. After osteotomy, patients noticed an improvement of their balance (p=0.014) and their gait (p<10-4). The postoperative lateral bipodal balance was worse for patients than for contrais (p=0.011). However, lt didn't exist any work fail or asymmetry support between groups. The monopodal balance was more stable from both sides, operated (p=0.011) and non-operated (p=0.009). About gait, only step width in postoperative had increased (p=0.001 ). After osteotomy, clinical scores had significantly improved, except for the underscore ASL of Koos. Clinical and perception scores would be more sensible than objective analysis to quantify a preoperative deficit and its 4 postoperative months improvement.
