Résultats à long terme de la reconstruction chirurgicale des ruptures de poulies sous cutanées chez les grimpeurs de haut niveau selon la technique de Lister

Abstract : Introduction: Sub cutaneous Pulley ruptures are almost the prerogative of rock climbers. The aim of the study was to evaluate the efficiency of surgical treatment for sports performances recovery. Materials and methods: 38 top-level climbers showing an operated pulley rupture were included. Assessments were made by recovering the climbing level, ultrasound measurement of the flexor tendons – phalanx space (E space), the measurement of the digital strength and of the range of motion. We compared our results with the conservative treatment by thermoplastic ring. Results: The follow-up was 7.1 years (0.9 to 17.3). 79% of patients recovered their climbing level. The tendon bowstring has been corrected in 58.1% of cases. The E space was reduced by 2 +/- 1.6 mm (p <0.01). 5 patients had a flexion contracture of the PPI (+ 16.4 ° +/- 3.85), and the overall amplitude of finger surgery was 97.2 +/- 6.1% of that of healthy side. The strength of the tip pinch was 101.7% +/- 10.3% of that of healthy finger. Practice in competition and surgical treatment before three months have given respectively 10.9 (1.4 - 82) and 15.1 (1.3 - 174) times more chances for patients to recover their level. There was no difference between the surgical and conservative treatment. Conclusion: Surgical treatment of pulley ruptures in high-level climbers allowed recovery of sports performances in the majority of patients, without consistency. Practice in competition and the delay before intervention was crucial for the recovery of climbing level.
