Reconstruction des oropharyngectomies robot-assistées par lambeau antérolatéral de cuisse fin

Abstract : Transoral robotic surgery (TORS) opened up new possibilities for the resection of oropharyngeal tumors. When a free flap reconstruction is needed the most frequently used flap is the radial forearm free flap. In this study, we evaluate the feasibility of oropharyngeal free flap reconstruction with free thin anterolateral thigh flap, elevated in the plane of the fascia superficialis after color Doppler sonography planning. Ten patients underwent TORS oropharyngeal tumor resection followed with free thin anterolateral thigh flap. There was one flap failure. Seven of the 9 patients whom flaps were successful, were not dependent of a feeding tube at the last follow-up. Four patients died during follow-up, two of them were still dependent of a gastrostomy feeding tube. All patients could be decannulated except one with a preexistent laryngeal stenosis. No locoregional recurrence was observed during follow-up. Thin anterolateral thigh flap is a valid alternative to radial forearm free flap for TORS oropharyngeal reconstruction. It decreases donor site morbidity in a trend toward minimally invasive surgery.
