Le bloc osseux personnalisé en chirurgie pré-implantaire

Abstract : Bone deficits in the jaws can make it impossible to position an implant on the edentulous ridge in an ideal situation, that is to say prosthetically guided in the three directions of space, which is a sine qua non condition for a long-term satisfying implant-supported rehabilitation at the biological, functional and aesthetic aspects. It may therefore be necessary to reconstruct this insufficient bone volume by preimplant surgical techniques. The bone block graft is one of the most commonly used bone augmentation surgeries in pre-implant surgery. The autogenous bone block, taken from the patient, remains the gold standard. Other materials are nowadays used and are of allogeneic, xenogeneic or synthetic origins. These alternatives to autogenous bone allowed a major evolution of the pre-implant apposition surgery: the personalization of the bone block from the initial radiographic examination. The surgeon then has a customized graft even before the surgical procedure. In this work, the conventional technique was studied, then an analysis of the literature was made to identify the principles, the different surgical techniques and the results brought by the surgeons using the modern technique of personalized blocks. These recent publications, mainly in the form of case reports, do not allow to dislodge the autogenic bone block from its place of gold standard. But the customization of the graft has some advantages over the conventional technique that make it a very interesting even promising technique for improving the oral rehabilitation of our patients.
