Optimisation de la prise de rendez-vous en cardiologie dans le cadre du parcours de soins coordonnés

Abstract : Background: Since the law of August 13th, 2004 that created the « coordinated healthcare process », the organization of healthcare professionals has to adopt an approach of healthcare process efficiency and optimization. However, friction still exists, amongst which the time needed to get appointments with specialists. Objective: The purpose of this thesis was to propose and evaluate a liaison card as a support to improve the process to get appointments with the cardiologist, the GP’s main interlocutor. Material and methodology: This is a prospective study performed over 6 months from December 2017 until July 2018. It took place in the Hautes-Alpes area, with 15 GP and 3 cardiologists participating. The process was as follows: the GP who wanted to refer a patient to a cardiologist would attach the liaison card to his mail, filled with the desired timing for the patient’s appointment. Four levels of urgency were defined. At the end of the study, a feedback questionnaire was sent to each of the professionals who participated. Outcomes: 49 patients were involved. 37 patients needed a relatively urgent appointment, and the desired timing was met for 73% of them. For 82% of the patients involved the timing was considered relevant by the cardiologist. 9 out of 11 GP who replied to the feedback questionnaire would like to use this process in the future. All 3 cardiologists who participated would also like to continue on these principles. Conclusion: This methodology can be included in a broader process where traditional means would blend with more innovative ones such as telemedicine. Amongst the leads that could be explored, the idea of being able to book appointments online using the same set of criteria depending on level of urgency and the same approach extended to other specialties seems very interesting.
