Le déni de grossesse : les représentations des sages-femmes influencent-elles la prise en charge des femmes ?

Abstract : Objective : The objectives of this study were to show that the representations of the denial of pregnancy were different from one midwife to another, and that these influenced their care (especially in psychology) by the mobilization of violent affects and the setting up of defense mechanisms. Methods : The type of study is a qualitative study based on ten semi-structured interviews with hospital or liberal midwives. These interviews consist of two parts: a part with free questions and a part composed of four clinical cases. Results : The results of this study showed us that the representations of midwives on the denial of pregnancy were not at all uniform. In addition, the analysis of lexical fields and responses to personal and professional feelings about denial prove us that the denial of pregnancy produce well and truly violent affects, which also led to the establishment of defense mechanisms such as projective identification, altruism or humor. Finally, the clinical cases allowed us to confirm that the care of these patients was modified, especially in the psychological care, with more frequent use of psychologists for example. Conclusion : The denial of pregnancy is a significant phenomenon midwives, implying the establishment of defense mechanisms, which are more important if the discovery of the denial is late.. This intriguing and surprising phenomenon also makes it necessary for midwives not to be alone in this care, especially with the help of psychologists.
