Apte à la plongée ? Oui, mais …

Abstract : Introduction: despite a strict regulatory framework including an annual medical visit and the signing of a medical certificate of no contraindication to practice (CACI), accidents and recreational diving mortality are increasing, particularly in "seniors" (> 50 years old). Material and method: a systematic review, analyzing 31 articles published in French or English after 2004, made it possible to specify the possible intrinsic or environmental risk factors favoring decompression sickness and pulmonary edema of immersion, exclusively in recreational diving, and to wonder about the authorization since 2014 of any doctor to sign this CACI. Results: the risk of accident during a recreational dive increases with age; there are more serious accidents and more sequelae after 50 years. Three intrinsic risk factors are age-related: increased prevalence of cardiovascular disease, higher body fat, and physiological senescence of the cardiovascular system. All ages combined, cold exposure and dive depth are risk factors for accidents. Conclusion: US diving agencies impose, regardless of age, a limitation of diving with air at 40 meters depth, excluding diving profiles requiring decompression stops. The FFESSM, less secure, allows these profiles, with a maximum authorized depth of 60 meters. These two approaches to recreational diving do not have the same fitness requirements. We suggest that the CACI written by a general practitioner not specialized in diving medicine concerns only "American-style" diving and the CACI dedicated to "French-style" diving requires the advice of a specialized doctor, especially after 50 years.
