Enquête de pratique autour de la consultation pour la délivrance du certificat de non contre-indication à la plongée loisir

Abstract : Introduction: recently, general practitioners were allowed to deliver non contraindication certificates (NCIC) for level 2 and higher divers. However, the universitary course doesn’t provide any specific learning. Primary objective tries to describe, according to the divers, the medical assessment of fitness to dive leading to a NCIC. Secondary objective describes the qualifying level of the physicians and their use of support documents during this consultation. Material and method: an anonymous paper survey was given to licensee divers on their leisure sites. Questions on socio-demographic data and on key-points of the consultation were asked. General and federal practitioner were surveyed on their qualifying level in the diving domain and their use of self-administrated survey or examination sheets. Results: 113 divers were included in the analysis. 65,5% of the certificates were written by the family physician, 8,8% by a general practitioner and 15% by a federal doctor. In 74,3% of cases, neither the self-administrated survey or the examination sheet were used. 29,2% of the consultations lasted less than 15 minutes. 20 physicians were included in the second analysis. Only federal doctors kept forming in the diving domain. 9 physicians didn’t use any support document. 3 Never refused writing a fitness to dive certificate. Discussion: most of the NCIC were written by general practitioners. However, continuing education offers in this domain is limited and doesn’t fit to the needs. Using self-adminstrated survey and examination sheets could be more systematic. The federation and the clubs could play a role spreading these documents. They would serve both medical and legal parts. A medical booklet that the diver inevitably presents during a diving-related consultation could be created. Conclusion : diving accidents can be dramatic. The evolution of the legislation concerning diving NCIC enforces to rethink the general practionner studies to this specific consultation.
