Influence de l'alliance thérapeutique dans la rééducation du patient adulte avec dysphonie dysfonctionnelle

Abstract : Background and objectives- The goal of this experiment consists of studying whether the evolution of the therapeutic alliance is correlated (or not) with the evolution of the voice throughout the rehabilitation of adult patients who have a dysfunctional dysphonia.
Methods-This observational and transversal experiment was carried out among a cohort of twelve patients we met in multiple independent speech therapists’ offices. On three occasions of their rehabilitation, we carried out a vocal evaluation (GRBASI, objective parameters and VHI) as well as a therapeutic alliance evaluation (WAI). The « therapeutic » part of the WAI evaluation was filled by the speech therapist.
Results : The VHI scores are the parameters with which the therapeutic alliance seems to be the most correlated. When the therapeutic alliance improves from one period to the other, at least one VHI criterion score decreases on average. When the WAI scores decreases, the vocal functional discomfort also increases. Our study did not find any correlation between the WAI scores and the objective parameters of the vocal evaluation. Neither did we find any predictive factors of what constitutes a good therapeutic alliance at the beginning of the rehabilitation.
Conclusion- This study shows a link between subjective feeling of their voice by the adult patients with dysfunctional dysphonia and relation which has been built with the speech therapist who treats them. Psycho-social and relational factors and efficiency of these reeducations are partially linked.
