Comparaison entre des médecins autofictionnels dans la littérature d’hier et d’aujourd’hui, et des médecins dans la vraie vie en 2019. Étude qualitative basée sur une recherche narrative avec analyse de contenu et des entre-tiens individuels semi-directifs avec 10 médecins en Région Auvergne

Abstract : Introduction : the characters of doctors in literary and television fiction have already been studied. However, we did not find a study directly comparing autofictional physicians in the literature of yesterday (20th century) and today, with physicians in real life in 2019. Methods: - narrative research with content analysis. 11 books from 9 medical writers were included for analysis. - qualitative study, based on individual semi-structured interviews with 10 doctors in the Auvergne Region, 9 of whom had already read literary accounts of doctors. Background and conceptual documentary research : in this section we present different concepts useful for understanding the study, explained through a non-systematic review of the scientific literature. Results : The autobiographical novels of physicians were compared to doctors in real life in 2019 based on seven themes of analysis: 1. The way the doctor gets in touch with his patient. 2. The ordinary suffering of the patient, and the role of psychologist of the doctor. 3. The mentors and models doctors. 4. The doctor-hero or savior. 5. Anti-model doctors and counterexamples. 6. The doctor, a human being like the others. 7. Attraction-repulsion in the doctor-patient relationship. Discussion : the autofictional doctors in the literature of yesterday and today and the doctors in real life in 2019 are similar on some points, differ on others. Similarities : variability of doctor-patient relationships, benevolent paternalism, asymmetry of the doctor-patient relationship, role of unofficial psychologist of the doctor. Differences : type of doctor-patient relationship, role models, perceived feelings of heroism, psychological and physical fragility of the physician, doctor-patient attraction-repulsion. Conclusion : The autofictional doctors in the literature of yesterday and today, and the doctors in real life in 2019 are not really so different. The narrative and clinical consultation are intimately linked.
