Évaluation de l'impact d'une discordance entre hormone anti-müllerienne et compte des follicules antraux sur la compétence ovocytaire en FIV ICSI

Abstract : Objective: Anti-Mullerian hormone (AMH) and antral follicle count (AFC) are usually defined as the best and comparable markers of ovarian reserve to predict ovarian response to stimulation during in vitro fertilization (IVF). Our study consisted in evaluating impact of discordance between AFC and AMH on oocyte competens and assessing its implcation on ovarian response in ICSI IVF. Materials and methods: This is a retrospective study on 536 womend undergoing the first ICSI IVF cycle at the Grenoble university hospital between January, 2015 and 2019. AMH and AFC discordance was defined according two cut-off ; respecively 1.1 ng/ml and 12. This allowed categorization subjects into four groups : two groups with concordant markers (nominate concordant « plus » if it’s above the cut-off and concordant « minus » if it’s below) and two groups with discordants markers (nominate « dAMHsup group » if AMH is above the cut-off and CFA below, and « dCFAsup group » for the opposite). Results: Women in concordant « plus » and AMH groups had a better ovarian response than concordant « minus » and CFA groups. Their numbers of retrieved mature oocytes and numbers of embryos was higher and fertilization failures were less frequently. The fertilization rate and ratio between embryos and retrieved oocytes were similar. Conclusion: When AMH and AFC are discordant, the ovarian responsiveness is intermediate, between the both concordant groups. Women who have AMH higher than AFC tend to respond better to stimulation. AMH would be a relible markers than AFC in case of discordance.
