Valeur pronostique du stroke volume indexé dans l'hypertension pulmonaire

Abstract : Introduction: in patients with pulmonary hypertension, right cardiac catheterization and cardiac ultrasound are realized both at the diagnosis and repeatedly during follow-up. Indexed stroke volume is obtained from cardiac index by right cardiac catheterization and its prognostic value has already been proved. Our aim was to compare an echocardiographic method to obtain the indexed stroke volume to the hemodynamic method and to analyze their prognostic value.
Material and method: data from the registry of pulmonary hypertension of CHU de Nice were taken into exam. Indexed stroke volume was obtained both by cardiac ultrasound and right heart catheterization. The two methods were compared. Correlation with mortality and transplantation was then analyzed.
Results: from July 2008 to November 2018, cardiac ultrasound and right heart catheterization data were available for 109 patients. For 73 of these 109 patients, the two exams were realized within a 30 days delay. Correlation between cardiac ultrasound and right heart catheterization regarding indexed stroke volume was weak with an r coefficient of +0.45. There was no correlation between echographic indexed stroke volume and risk of mortality or transplantation (p=0.2467); on the contrary, in multivariate analysis, hemodynamic indexed stroke volume had a prognostic value (p=0.0226).
Conclusion: we could not find any agreement between cardiac ultrasound and right cardiac catheterization in the measurement of indexed stroke volume. Indexed stroke volume, as obtained with right heart catheterization, was a risk factor for death and transplantation.
