Évaluation du niveau de précarité des patients atteints de coronaropathie et chez des patients artériopathes : étude PRECARIEN

Abstract : Introduction: Peripheral arterial disease and coronary heart disease are a global health issue with their consequences in terms of morbidity and mortality.Socioeconomic deprivation is a risk factor for cardiovascular diseases.According to a previous study, patients with coronary heart disease have a higher level of education than patients with peripheral arterial disease. The objective of this study is to measure and the deprivation level of patients suffering from symptomatic peripheral arterial disease and those suffering from isolated coronary heart disease using a global approach score, then compare them. Methods: This is an observational, cohort, prospective, multicenter study. Patients were recruited from vascular or cardiac rehabilitation departments, vascular medicine or vascular surgery departments and in cardiology department. The EPICES score and the INSEE parameters were used for the analysis.Cardiovascular risk factors and PAD stage were collected. Results : 230 patients were included. 47.8% of patients with symptomatic PAD were in a precarious situation compared to 17.4% of patients suffering from isolated coronary heart disease, according to the EPICES score (p <0.001). Socioeconomic deprivation was associated with smoking (p <0.001), diabetes (p <0.001) and a younger age (p = 0.02). More the PAD is severe more the level of deprivation increased (p <0.001). Conclusion: Patients suffering from PAD are more precarious than patients suffering from coronary heart disease. Mechanisms linking PAD to deprivation are not all known at present. A better detection of socioeconomic deprivation in patient suffering from PAD could allow to comprehensive care and thus hope for an improvement in terms of morbidity and mortality.
