Le ressenti des étudiants en maïeutique face à la simulation haute-fidélité et l’hémorragie du post-partum immédiat

Abstract : Context: high-fidelity simulation provides clinical experience in dealing with serious but rare obstetric emergencies such as postpartum hemorrhage. This pathology causes the death of 1.2 patients per 100,000 live births and in the majority of cases they are preventable. The simulation showed its interest in improving non-technical skills that would allow optimal management of these patients. Objective: the main objective of our work is to study the feelings of midwifery students during high-fidelity simulation sessions on the theme of postpartum hemorrhage and their feelings about this pathology. Our secondary objective is to map initial training by high-fidelity simulation on the theme of postpartum hemorrhage through French midwifery schools. Population and method: we carried out a quantitative cross-sectional observational study using anonymous questionnaires, distributed to final year students in the maieutic sciences, promotion 2015-2019, from 35 schools of French midwives. Results: we obtained responses from 21.2% students (n= 215/1012) from 33 midwifery schools. High-fidelity simulation sessions on the theme of postpartum hemorrhage are offered by 84.8% (n = 28/33) of schools. The majority of participants in our study (74.5%) declared that they were stressed during the session. This feeling negatively impacted their feelings for taking charge of postpartum hemorrhage (p <0.001) and their post-session self-confidence (p = 0.001). The role of actor during the training allowed better communication (p = 0.020). Student confidence during the session was reported to be good in only 22.4% (n = 37/165) of participants. In total, 73.5% of students reported a positive feeling to diagnose and manage postpartum hemorrhage. Practical work (p = 0.025) and high-fidelity simulation (p = 0.001) were the only factors found to influence this positive feeling. The multidisciplinarity during the simulation sessions and the repetition of this teaching were the perspectives proposed by the students. Conclusion: our study shows the interest of high-fidelity simulation on postpartum hemorrhage, for the acquisition of non-technical skills and the improvement of the feeling of students to take charge. It would be interesting to study whether the skills acquired during a session on a given theme can be applied to all emergency situations.
