Facteurs influençant les délais d’admission des patients admis aux urgences de Pau pour un accident vasculaire cérébral ischémique : étude épidémiologique descriptive

Abstract : Background: ischemic stroke is accessible to thrombolytic therapy if it is started within 4.5 hours of symptoms onset. Many patients can’t have access to it because of delayed consultation. Lack of awareness about the stroke emergency is the main responsible. The objective was to study the factors influencing the delays in admitting patients with ischemic stroke to the emergency of Pau. Methods: it was a retrospective epidemiological study including patients consulting for ischemic stroke from 2018 to 2019. Data collected were: deficit onset; age; sex; address; cardiovascular risk factors; history of stroke and heart attack; antiaggregant treatment; autonomy assessment; method of seeking care; neurological symptoms; NIHSS score. Two groups were formed: group 1, patients with an admission deadline ≤ 4.30 hours; group 2, patients with an admission deadline> 4.30 hours after the deficit onset. Results: among 458 files analyzed, 381 patients were included, 217 (52%) presented within ≤ 4.30 hours of symptoms onset, 164 (48%) after 4.30 hours. The median time to consultation was 3.50 hours [1.5-10]. In multivariate terms, the factors associated with a late admission delay were: assistance with autonomy (OR = 2.62), emergency consultation by private means (OR = 5.98), recourse to liberal medicine (OR = 17.66), ataxic (OR = 2.7) or visual (OR = 2) deficit. Discussion: these results could allow to target populations at risk, and put in place adapted means of prevention and education in stroke.
