Expression clinique comparée du trouble de l’usage d’alcool en fonction du sexe. Analyse des données de l’enquête transversale « Santé Mentale en Population Générale », France, 1999-2003

Abstract : Background and objective: Epidemiological and biological arguments highlight physiological, behavioral and comorbid differences between men and women suffering from alcohol use disorder (AUD). The Mental Health in General Population (MHGP) database is derived from a representative national survey of French adult population (n=39,590), conducted between 1999 and 2003. The presence of AUD was assessed using the Mini International Neuropsychiatric Interview (MINI). In our present study, we compared the symptom patterns of AUD among men and women with AUD respectively. Methods: In the sample without empty data, a total of 892 subjects met the criteria for AUD, among them, 725 (81.2%) men and 167 (18.7%) women. Each of the nine AUD criteria of the MINI were compared with socio-demographic characteristics and psychiatric comorbidities. Influence of sex on the clinical patterns of AUD were determined using multiple logistic regression models. Results: With the males’ group as the reference, the occurrence of accident (car accident or with a knife or with another engine…) were the only one of the nine AUD criteria of the MINI associated with sex, with an adjusted odds-ratio on socio-demographic characteristics and psychiatric comorbidities of 0.62 (95%CI, [0.43 - 0.91]). Conclusion: the clinical expression of AUD does not differ as much depending on the sex. However, being a man with AUD is associated with a higher occurrence of accidents. These results showed the good homogeneity of the MINI questionnaire which allows the identification of a TUAL regardless of sex.
