Étude du transfert de compétence médecin-infirmier à travers la pose de voies veineuses périphériques échoguidées

Abstract : Introduction: Peripheral intravenous line placement is a common procedure in Emergency Departments (ED). However, a large number of specific situations within the Indian Ocean population, make this procedure difficult. We found it essential to offer nurses an alternative: an ultrasound guided procedure. Methods: We proposed a specialized training session for the EN of the Northern Réunion and Mayotte hospitals, after validation and approval by the ARS and HAS via the COOP-PS « The making of scans of veins and/or arteries of upper members by a nurse instead of a doctor ». The theoretical training and practise on phantoms was completed by the supervision of the first 15 acts in the form of mentoring. Nurse skill progression was evaluated by pre and post training questionnaires, as well as the analysis of the first 15 supervised real life acts. Results: During the period from 29th March 2019 to 13th October 2020, we trained 32 nurses in total. At the end of the training period, 75 % sufficiently mastered the act and 71.8 % mastered the knowledge of the upper member. Finally, 78.2 % of the nurses felt in control and 93.3 % considered that the training was sufficient. After 10 ultrasound guided peripheral intravenous cannulations, the nurses managed to infuse after an average of 1.2 tries and no longer showed signs of anxiety about this act. It took 12.3 minutes in all, including the settling down of the patient. Conclusion: This training session appeared to be efficient for the majority of nurses who improved their skills in all the taught areas. This program answered a specific need in our ED by updating practices and improving autonomy in our teams, while freeing up medical.
