Utilisation de l’échographie cardiaque dans l’état de choc aux urgences : aide thérapeutique, analyse rétrospective

Abstract : Shock is a vital emergency for the patient. Identify etiology, quickly, is a challenge in emergency department. Current recommandations advise the use of ultrasound in the management of shock to diagnostic and therapeutic assistance. In this study, we collected retospective patients database in emergency deparment in Saint-Lô from 01 january 2018 to 31 december 2019. Patients were analyzed in two groups: those who had echocardiography by emergency doctor and those who did not have this examination. Our primary objective was evaluate the contribution of echocardiography to resuscitation fluid. Also, we compared the use of catecholamine and its type. We found an absence of difference in the use of the resuscitation fluid but a significant difference in the use of catecholamine in patients who received echocardiography. Indeed, echocardiography identificate the etiology of shock, its physiopatology and vasoplagia to hypovolemia. Nevertheless, the echocardiography is underused, only 26% of patients have ultrasoud and mainly those with an electrocardiogram leading to a state of cardiogenic shock. Our study reveal that more frequent use of echocardiography is beneficial for all patients in shock.
