Obstacles d’une fin de vie à domicile de patients en soins palliatifs : étude qualitative menée auprès de 15 aidants en Guadeloupe

Abstract : Introduction : Care network organization in our region responds to archipelagic and medical demography barriers. Despite the demand of people to die at home, the reality is quite different. In France, 8 million caregivers and more represents for patients, strong link, fragile and sometimes suffering. Through their testimonies, we look for circumstances their loved ones dying at the hospital Objective : Inventory of medical ambulatory for palliative patients and identification of failure to die at home. Material and method : Qualitative study with referring caregivers of patients living at home and dead in emergency rooms of Guadeloupe University Hospital in 2018. Results : 15 semi-direct interviews were conducted on average of 39 minutes. Ignorance of palliative status of their loved one represents the large majority. Forgetfulness of the generalist was futile. The lack of anticipation has been translated by the suffering of the caregivers, their scared noted their death, and the hope of surviving them "once more". Even if they noted the worsening of the state of health, death’s medicalization appeared as an obvious solution, the only one. Even though their close wish was the opposite. Death has bent the strongest wills. Conclusion : The place of city-hospital network appears as proof, to minimize the emotional scars of post-mortem caregivers.
