Parcours de soins de fin de vie des patients suivis par l'équipe mobile de soins palliatifs au domicile et décédés pendant l'année 2018 sur le territoire du Béarn et de la Soule : étude longitudinale sur une cohorte historique dans le territoire du Béarn et de la Soule du 1er janvier 2018 au 31 décembre 2018

Abstract : Introduction: most people want to spend the last moments of their lives at home. However, nearly 60% of deaths occur in the hospital. EMSP could then improve this figure in favour of extended home stay. Objectives: the main objective is to determine the rate of patients, followed by EMSP at home in the territory of Béarn and Soule, maintained at home 28 days, 7 days and 24 hours before the death as well as the day of the death. We also assessed the rate of hospitalizations and emergency department visits, as well as various factors that could be a barrier to stay home. Method: longitudinal study on a historical, monocentric cohort. Collection of different information from the 76 patients monitored at home (medical-social institutions or own home) by the EMSP of the hospital of Oloron, and deceased during the year 2018: general characteristics of the patient, data concerning the follow-up and characteristics of the death. Results/discussion: The majority of patients spent the last days of life in their homes. The patient’s wish for the place of death was respected in 80% of cases. During the follow-up, emergency department visits were also reduced, with a decrease in the length of hospitalizations. The distress of caregivers, the unequal access to GAD financial assistance and the often late management of palliative care patients can make it difficult to keep them at home. Conclusion: thanks to the support of the mobile team, patients were able to stay as long as possible at their home during the end of their life, as they wished, and often until death. Our analysis highlighted the importance of continuing to develop the palliative culture. It is also important to improve support for patients and their caregivers, as well as to ensure equity in access to palliative care.
