La pulpotomie coronaire à visée définitive des dents permanentes matures vitales avec ou sans pulpopathie, associée ou non à une atteinte péri-apicale : une revue systématique de la littérature

Abstract : Aim: a better understanding of the pulp healing phenomena has allowed the definite coronal pulpotomy, to gain in importance in recent years. The aim of our study is to examine the clinical and radiological success of the partial or total pulpotomy of the vital mature permanent teeth with pulp exposure of carious origin, with or without pulpopathy and/or periapical involvement and to assess whether it can be considered as a customary treatment. Material and methods: this is a systematic review following PRISMA recommendations, using the PubMed, DOSS and Scopus databases. The informations have been selected on a primary reading of the titles and the abstracts, which then led to a full reading of the articles. Studies meeting inclusion and exclusion criteria were included. The bias evaluation has been made for each study individually, using the criteria developed by the Cochrane Collaboration. Results: 12 articles have been included in the revue. Their studies present a rate of success of the definite coronal pulpotomy of almost 90%. The MTA, CEM and Biodentine seem to be the leading pulpotomy agents with even greater results than Calcium hydroxyde. Conclusion: the definite coronal pulpotomy of the vital mature permanent teeth can now be considered as an efficient alternative for ECR. However, both larger scales studies and a follow up of patients over a longer period would be necessary in order to one day make this treatment a standard procedure.
