Efficacy and safety of hyperbaric oxygen therapy monitored by fluorescein angiography in patients undergoing a retinal artery occlusion

Abstract : Objective: to assess the efficacy and safety of a standardized Hyperbaric Oxygen Treatment (HBOT) protocol monitored by fluorescein angiography (FA) in patients with Retinal Artery Occlusion (RAO).
Design: prospective, non-comparative, and unicentric study from July 2016 to March 2021.
Participants: patients with RAO lasting from less than 7 days without cilioretinal artery preservation or previous treatment.
Methods: all consecutive patients diagnosed with RAO underwent a FA, a macular Optical coherence tomography (OCT), and OCT-Angiography (OCT-A) followed by two daily HBOT sessions (2.5 ATA, 90 min) until revascularization assessed by FA occurred. Complete ophthalmic follow-up was scheduled at day 14, day 21 and at one month.
Main outcome measures: Best Corrected Visual Acuity (BCVA) improvement defined as a decrease ≥0.3 logMAR at one month.
Results: thirty-one patients were included and underwent an average of 33.9 HBOT sessions. Retinal revascularization was diagnosed in 48.4% and 87.1% of the patients at day 14 and day 21, respectively. The mean BCVA at referral and at 1 month were 1.51 logMar and 1.10 logMar, respectively. Fifteen (48.4%) patients fulfilled the main outcome measure. Six (19.4%) patients suffered from minor barotrauma without requiring HBOT withdrawal. OCT-A was not suitable enough to assess retinal occlusion or revascularization.
Conclusion: HBOT, monitored by FA until spontaneous revascularization of the central retinal artery occurred, was both effective and safe. OCT-A was not reliable for establishing the diagnosis of RAO and subsequent revascularization.
