Acceptabilité des anticoagulants oraux chez les personnes de plus de 65 ans à Paris

Abstract : Introduction - Acceptability of drugs has been identified, as one of the factors of medication compliance. On the initiative of the AGEPS (Agence générale des équipements et produits de santé), a working group with the partnership of the company ClinSearch, was able to set up a large observational study on the acceptability of drugs. An ancillary study conducted at Broca hospital was to evaluate the acceptability profile of oral anticoagulants for patients over 65 years old. The secondary objectives were to compare the acceptability profiles according to the presence of geriatric syndromes as well as that of AVK vs DOAC. Methods - All patients over 65 with anticoagulant therapy agreeing to the study were included. A standardized questionnaire according to the CAST method was provided after the hetero-evaluation of the drug intake. Multiple correspondence analysis defined two acceptability profiles (accepted / not accepted). Results - Over 5 months, 171 patients (including 120 women) were included, with a mean age of 86.5 years, 139 living at home, 135 with cognitive impairment, 24 with swallowing disorders and 122 with were fragile. The treatments taken were rivaroxaban (n = 73), warfarine (n = 61) and fluindione (n = 35). In 84% of the observations, the profile was "accepted". The presence of geriatric syndromes was statistically associated with lower acceptability. The acceptability profile was better if the patient took his treatment alone. DOAC tended to be better accepted than VKA. Conclusion - Anticoagulants are well-accepted treatments for people over 65 years old.
