Impact de la génioplastie fonctionnelle chez le patient en période péripubertaire. Revue systématique de la littérature

Abstract : Genioplasty is proposed in most cases to adults as part of orthognathic surgery. It brings a real benefit both aesthetic and functional but when this genioplasty is isolated, it can be indicated to correct a vertical mandibular excess without excessive divergence of the mid facial height, mostly in addition to orthodontic treatment, at the end of it. When it is performed early before the end of growth, it is qualified as "functional". The ideal age for this procedure is a controversial topic. The improved facial appearance and positive psychosocial aspects would tend to an early intervention for patients with the most severe deformity. Concerns about a possible negative effect on mandibular growth or therapeutic stability would be in favor of waiting for the end of growth. In this context, a review of the literature was carried out to study whether, done at the opportune moment and with sufficient displacement amplitudes, functional genioplasty makes it possible to obtain a vertical closure improving the therapeutic result and its stability in hyperdivergent patients. Four articles demonstrated the positive consequences of functional genioplasty on the direction of growth as an anterior rotation of the mandible and the improvement of ventilatory parameters in growing patients with isolated anterior vertical excess and ventilatory and labial dysfunctions. The studies showed that the results remained stable during the follow-up period ranging from 6 to 24 months. No difference in symphyseal growth was reported in studies between operated patients and the control group. Therefore, genioplasty appears to be a mean of ensuring the aesthetic, functional and stability goals of orthodontic treatment by redirecting the patient's growth, without altering it.
