Artérites à cellules géantes diagnostiquées par une ischémie cérébrale, description monocentrique de 5 cas et revue de la littérature

Abstract : The aim of this study was to determine the clinical, radiological and evolutive presentation of patients who suffered from a stroke that led to the diagnosis of giant cell arteritis (GCA). We used a monocentric database of GCA patients, from 2010 to 2020 in Basse-Normandie, and retrieved 5 from 319 (=1.7%) patients who had a stroke at the time of GCA diagnosis. We also performed a literature review, by looking up in Pubmed for "GCA" and "stroke". We identified 14 articles (159 patients). There were 5 men in our series (median age 72 years), with vision loss and involvement of the vertebro-basilar territory in 80% patients, respectively. There was a polymyalgia rheumatica in 40% cases. Two patients had a severe disease with multiple stenosis, and early stroke reoccurence. The literature review showed a male/female ratio at 1/1. The mean age at diagnosis was 76,9 years old. In 78% of cases, GCA was diagnosed at the time of stroke. There was a vision loss in 64% patients, and a polymyalgia rheumatica in 22% of cases. The following factors were found to be more significantly present in stroke GCA patients: male sex, older age, the absence of polymyalgia rheumatica, a less important inflammatory syndrom, the absence of anemia, a vision loss caused by GCA. All patients received glucocoritoids, eventually preceded by intravenous solumedrol boli. Poor outcomes were observed in 28% of patients who died.
