Troubles anxio-dépressifs à distance d'une épidémie de Chikungunya: étude exposé-non exposé ancillaire à la cohorte réunionnaise CHIKGene

Abstract : Background : Anxiety-depressive disorders (ADD) have been little studied as consequences of Chikungunya virus infection (CHIKV). The primary objective was to authenticate among subjects exposed to CHIKV, either CHIK+ disabled (chronically painful or exhausted) or CHIK+ healthy (free from pain or fatigue), the presence of anxiety, depression or mixed ADD in comparison of an unexposed population (CHIK-), at distance from the epidemic that occurred in 2005-2006 on Reunion Island. Method : Among the participants of the cohort of the CHIKGene genome-wide association study, we compared anxiety (HAD-A) and depression (HAD-D) scores estimated from the Hospital Anxiety and Depression Scale questionnaire (HADS), according to exposure, distinguishing four phenotypes: neither anxious nor depressive (HAD-A and HAD-D≤10), anxious but not depressive (HAD-A>10 and HAD-D≤10), depressive but not anxious (HAD-A≤10 and HAD-D>10), anxious and depressed (HAD-A and HAD-D>10). Results : Four hundred and fifty-one subjects were enrolled including 201 CHIK+ disabled, 178 CHIK+ healthy and 72 CHIK-. Of these, CHIK+ disabled subjects were more likely to have mixed ADD (25.4% versus 3.9% in CHIK+ healthy subjects or 4.2% in CHIK- subjects, p<0.001, respectively). The CHIK+ disabled subjects had higher mean HAD-A and HAD-D scores (9.8±4.2 and 8.1±3.7) than the CHIK+ healthy subjects (6.8±3.8 and 5.1±3.1, p<0.001, respectively) or the CHIK- subjects (7.8±3.8 and 6.0±3.4, p<0.001, respectively). Conclusion : ADD were more frequent and deeper in subjects who have presented protracted manifestations of Chikungunya than among those who have recovered or in unexposed subjects.
